Center for Economic Democracy Request for W9 Form - for Contractor Payment
In order to process payment for your consulting work with the Center for Economic Democracy (CED) or our fiscally-sponsored projects, we need your completed, signed, and dated Form W9 on file. Please follow the instructions in this request to upload your W9 securely to CED's DropBox folder. (This is where CED stores materials with sensitive Personally Identifiable Information. Access to the materials you upload here is restricted to only members within the CED team involved in payment processing.)
